Risk Insights (formerly Red Flags)
Risk Insights is a comprehensive reporting feature designed to help organizations proactively identify, monitor, and respond to potential compliance, safety, and workplace issues found within open-ended survey responses.
This feature, previously known as Red Flags, has been renamed Risk Insights to reflect its enhanced capabilities. While Red Flags highlighted responses for specific survey questions, Risk Insights offers a more robust and flexible way to monitor potential risk across your survey data. It allows easier setup, customizable filters, and automated delivery options, making it simpler to track and act on insights at scale.
Risk Insights allows administrators to continuously review sensitive feedback by combining compliance-focused questions, AI-driven analysis, and keyword monitoring.
Where to Find Risk Insights
Risk Insights is available under the Report and Comments pages in reporting.

Risk Insights is represented by a flag icon. Selecting this icon will allow eligible users to create Risk Insights reports. If you do not see the flag icon, this means you do not currently have access to Risk Insights.
What Risk Insights Does
Risk Insights monitors open-ended responses using three complementary methods. Each method captures risk signals in a different way, providing broader coverage and reducing the chance that important feedback is overlooked.
- Compliance Questions provide responses to specific, compliance survey questions that are designed to surface responses related to policy violations, misconduct, harassment, discrimination, safety concerns, or other workplace risks. These responses are isolated into their own section of the report to ensure visibility and appropriate handling.
- Key Words allow administrators to define specific words or phrases that should trigger a flag whenever they appear in an open-ended comment. This is especially useful for organization-specific language, internal terminology, names, locations, or topics that require immediate attention.
- AI Flagged Comments uses an AI model to analyze the language, tone, and context of open-ended responses. The model automatically flags comments that may indicate alarming behavior, sensitive content, or potential compliance risk, even if specific keywords are not present.
Risk Insights reports can be delivered automatically on a daily, weekly, or monthly schedule, ensuring that the right stakeholders stay informed without needing to log into the platform.
Setup & Configuration
1. Survey & Dates
The Survey & Dates section defines which surveys and responses are included in your Risk Insights report. These settings apply across all components of the report.

Name Your Report
When creating a Risk Insights report, you will first assign a report name. This name:
- Appears within the platform
- Is used as the title for emailed reports
Choose a clear and descriptive name so recipients can easily identify the purpose of the report.
Survey(s)
Only surveys that contain open-ended questions are eligible to be included in a Risk Insights report. Select the surveys you want to include in your Risk insights report.
The option “All surveys – Past, Present, and Future” allows you to automatically include:
- All existing surveys that contain open-ended questions
- Any surveys created in the future that include open-ended questions

With this option enabled, newly created surveys that meet the criteria will be added automatically and monitored without requiring updates to the report. This is ideal for users who want a long-term, set-it-and-forget-it approach to risk monitoring.
Date Range
Select the date range for data that should be included in your Risk Insights report. Depending on the survey type and selection, you may have preset options such as “last month” or the ability to apply a custom date range.
Demographic Filters
Demographic filters allow you to narrow the report to responses from specific participant groups. Any demographic filter applied here affects the entire report across Compliance Questions, AI Flagged Comments, and Key Words.
Demographic filtering is useful when monitoring risk within specific populations, departments, or segments of your organization.
2. Compliance Questions
Compliance Questions are only available if any of the People Element library compliance questions are included in your survey. Surveys with compliance questions included are marked with an orange flag.
Compliance Questions from Library:
- Do you know of any compliance or integrity issues that have not been addressed with the organization (e.g., improper, unethical or unlawful behavior)? If yes, please explain.
- Have you ever been asked to engage in conduct that you believe to be either unethical or illegal? If yes, please explain.
- Have you ever witnessed any conduct by an employee, volunteer, contractor or agent that you would characterize as either unethical or illegal?
- If yes, please describe what you witnessed
- (Answer only if you responded to the previous question) The organization takes these issues seriously and would like to be made aware of them. We will be forwarding this issue/s on. As much information as you can share will be most helpful (i.e., department, location, job titles, etc). All your other answers to this survey remain confidential. Do you give permission to identify your name with this specific comment? If yes, this will give the organization an opportunity to follow up and you may be contacted for more information.

When toggled ON, responses to compliance questions are populated in your report. Compliance question responses appear only in the Compliance Questions section of the report, and do not populate in the AI Flagged Comments or Key Words sections.
Responses to Compliance Questions are only accessible via a Risk Insights report or Raw Data and are available immediately after submission.
3. AI Flagged Comments

Once toggled ON, AI Flagged Comments use an AI model to analyze all open-ended survey responses to any open ended question in your survey (excluding compliance questions). The model evaluates language patterns, sentiment, and context to identify comments that may suggest:
- Alarming or threatening behavior
- Sensitive or distressing content
- Potential compliance or workplace risk
This approach allows Risk Insights to detect issues that may not be captured by predefined questions or exact keyword matches.
Permissions & Availability
To enable AI Flagged Comments:
- The user must have Comments access in their role permissions
- The feature must be included in your paid plan
If AI Flagged Comments are not available in your account, please contact your People Element representative to discuss adding this capability.
4. Key Words
The Key Words section flags responses when a specified word or phrase appears in an open-ended comment. This provides direct and transparent monitoring for terms that are especially important to your organization.
Key Words are commonly used to track:
- Organization-specific terminology
- Locations or facilities
- Topics that require immediate review

Managing Your Key Word List
You can manage your list of key words and phrases in several ways:
- Manually enter individual words or phrases
- Use the Export button to download your existing list
- Use the Import button to upload a list of key words in bulk

When importing key words:
- Instructions can be found on the first tab of the export
- Duplicate entries are automatically removed
- Key words and phrases are not case sensitive, ensuring consistent detection
Permissions & Availability
To enable Key Words:
- The user must have Comments access in their role permissions
- The feature must be included in a paid plan
Trial Mode
If Key Words are not included in your paid plan, a limited trial experience is available:
- Up to 5 key words or phrases may be added
- Only one Risk Insights report per user may use key words
This trial mode is intended to allow users to evaluate the value of Key Words before upgrading. To enable full functionality, contact your People Element representative.
5. Schedule For Delivery
Risk Insights reports can be delivered automatically on a recurring schedule (daily, weekly, or monthly) and can be sent to one or more recipients for each report.
Because Risk Insights contains sensitive information, it is important to:
- Only include recipients who are authorized to view this data
- Avoid adding shared inboxes or external email addresses unless appropriate
If no new responses meet your report criteria since the previous delivery, the report delivery email will indicate: “No new responses since the last report.”
Export & Report Format
Report File Structure
Risk Insights reports are delivered as an XLSX (Excel) file. Depending on how the report is configured, the file may contain up to three tabs:
- Compliance Questions
- AI Flagged Comments
- Key Words
Important Notes
- Compliance question responses appear only in the Compliance Questions tab
- AI Flagged Comments and Key Words appear only in their respective tabs
Data Processing Schedule
To ensure consistent performance while processing large volumes of open-ended responses, the system processes AI Flagged Comments and Key Words every 4 hours starting at 2am MST.
What this means:
- New responses may take up to 4 hours to appear in AI Flagged Comments or Key Words, depending on submission time
- Compliance question responses are available immediately
- When setting up a Risk Insights report for the first time, AI Flagged Comments and Key Words may not be available until the next processing window
Favorites
Any saved Risk Insights report can be accessed from the Favorites page within Reporting.
From Favorites, users can:
- Open and review saved reports
- Make changes to report settings
- Delete reports
- Copy existing reports
When copying a Risk Insights report, email recipients must be re-entered. This safeguard helps prevent sensitive reports from being sent to unintended recipients.
Other Considerations
Manager 360 Surveys
Manager 360 surveys can be included alongside other surveys in a Risk Insights report. However, there are important limitations to keep in mind.
Because Manager 360 participants differ from standard survey participants:
- Demographic data is not available for these surveys. Applying demographic filters to a report that includes Manager 360 + non-Manager 360 surveys will exclude responses from Manager 360 surveys.
- If your Risk Insights report includes only Manager 360 surveys, demographic filtering will not be available.
